Introduction

These Terms and Conditions (“Terms”) apply to your participation in the Onform Trial (“the Trial”) operated by Future Origin Labs Ltd, trading as Onform (“Onform”, “we”, “us”, or “our”). By accepting an invitation to participate in the Trial, you agree to be bound by these Terms, which are designed to protect both you and us and ensure a safe, fair, and legally compliant trial process.

If you do not agree to these Terms, you must not participate in the Trial or use the Trial product.

1. Eligibility

To participate in the Trial, you must:

  • Be aged 18 years or older

  • Be a resident of the United Kingdom

  • Have received a direct invitation from Onform to take part in the Trial

  • Not have any known allergies or intolerances to the listed product ingredients

  • Be the sole intended user of the product — it must not be given or shared with anyone else

Participation is personal, non-transferable, and subject to these Terms.

2. Exclusions

The following individuals are excluded from participation in the Trial:

  • Anyone under the age of 18

  • Anyone who intends to give the product to someone else, including minors

  • Individuals with known allergies, intolerances, or medical conditions that may be adversely affected by the product

  • Individuals who are pregnant, breastfeeding, or under medical supervision unless cleared by a qualified healthcare professional
    Anyone who has not received a direct invitation from Onform

  • Anyone who has previously breached Onform’s terms or policies

We reserve the right to disqualify any Participant, including after the Trial begins, if we believe these criteria have not been met or if participation poses a health or reputational risk to Onform.

3. Nature of the Trial

  • The Trial involves receiving one month’s supply of a pre-market food supplement product at no cost.

  • No payment, purchase, or subscription is required to participate.

  • Participation does not create any employment, partnership, agency or other commercial relationship between you and Onform.

4. Product Use and Medical Disclaimer

The Trial product is a food supplement. It is not intended to diagnose, treat, cure or prevent any disease.

You confirm that you:

  • Will not exceed the recommended daily intake

  • Will keep the product out of reach of children
    Are aged 18 years or older

The product is not suitable for:

  • Children or adolescents under 18
    Pregnant or breastfeeding women

Please consult your doctor before use if you:

  • Are under medical supervision

  • Are taking medication

  • Are taking other supplements containing zinc

  • Have concerns about active compounds such as ginseng, mushroom extracts, or other nootropics

Additional usage and safety information may be included on packaging.

5. Participant Responsibilities

By participating, you agree to:

  • Personally use the product as the invited participant

  • Not share, sell, or transfer the product to anyone else

  • Follow all instructions provided

  • Provide feedback on your experience with the product when requested, through surveys or other channels provided by Onform

  • Avoid making false, misleading, or defamatory statements about Onform or the Trial product

  • Comply with all other sections of these Terms

6. Ownership of Feedback

Any feedback, suggestions, or content you share during the Trial will be considered non-confidential and become the property of Onform.

You grant Onform a perpetual, royalty-free licence to use, adapt, and distribute such content in any format, including for marketing and research.

You waive any rights to review, approve, or be compensated for how it is used.

7. Intellectual Property

All rights in the Trial product, brand assets, and related materials remain the sole property of Onform.

You may not reproduce, share, or repurpose any Onform content without written permission.

8. Prohibited Conduct

You may not:

  • Sell or distribute the Trial product

  • Misrepresent your connection to Onform

  • Use the product unlawfully or for unauthorised purposes

  • Attempt to analyse or reverse engineer the product

  • Engage in behaviour that harms Onform’s brand, reputation, or trial integrity

9. Data Protection and Privacy

Your data will be handled in accordance with UK GDPR. We collect it to administer the Trial, analyse results, and share relevant updates.

10. Marketing Communications

By joining the Trial, you agree to receive relevant updates and marketing communications under the “soft opt-in” legal basis.

11. Limitation of Liability

Onform is not liable for indirect, incidental, or consequential damages related to participation or use of the product.

Our total liability will not exceed the estimated retail value of the product provided.

Nothing in these Terms excludes liability for death or personal injury caused by negligence or any liability that cannot be excluded by law.

12. Right to Modify Terms

We may update these Terms at any time. Continued participation implies acceptance of any changes.

13. Website Disclaimer

We are not responsible for third-party content, platforms, or tools linked from our site or communications. These are for convenience only.

14. Governing Law

These Terms are governed by the laws of England and Wales.
Any disputes will fall under the exclusive jurisdiction of the courts of England and Wales.
